Radiograph of the chest in a formerly premature neonate shows findings of the left pulmonary interstitial emphysema. There are bubbly branching lucencies scattered throughout the left lung. A moderate left pneumothorax is present. The pneumothorax is under tension causing displacement of the heart and mediastinum towards the right. Pulmonary interstitial emphysema is associated with prematurity, very low birth weight, and mechanical ventilation. Because the neonatal lungs are stiff and oxygen is delivered under pressure, air can dissect along the pulmonary interstitium. Potential thoracic complications of pulmonary interstitial emphysema include pneumothorax and pneumomediastinum.](https://app.figure1.com/case-detail/1ac97126-f40b-4447-b05e-7fd4db7f8342)
