This small bone fragment from the lateral aspect of the tibial plateau on a knee AP view is the classic appearance of a Segond fracture. It will mostly be caused by falls or sport injuries involving internal rotation and varus stress of the knee. It is very important to remember that Segond fractures are associated with an ACL rupture in almost 100% of cases, and with a medial (or sometimes lateral) meniscal tear in up to 75% of cases. An additional MRI is therefore always needed. Image credit: @keburley. Caption credit: @fel-x.
