Case Summary
31 y/o Male came in due to palpitations and chest discomfort that came on during exercise. Pt sts he also had marijuana prior to episode. Pt sts it improved with rest, now no discomfort or palpitations in ED. Pt reports similar prior episodes in the past year. Pt reports hx of father with heart problems, sudden death around 50. Pt hx of HTN recently put on lisinopril and has been controlled. Pt used cocaine “once or twice” a few years ago, not currently. No other cardiac risk factors, though pt just recently established primary care. No other complaints. This is his EKG, he did have a peripheral white count of 17.32 with slight left shift, cxr was without cardiomegaly or any other findings, thc+ though no other drugs. No prior EKG, repeat EKG unchanged, serial troponin negative.

- 21 YOM PT with no cardiac history reported sudden onset of palpitations after a postural change(orthostatic). Palpitations accompanied with SOB and dizziness-denies CP. Pt also febrile. Admits to marijuana use hours prior but is an experienced user and does not have adverse reactions. No hard drug use.
- 35 y/o male presents to EMS following a syncopal episode at groundbreaking ceremony. No prior Hx of syncope or cardiac problems. No Hx or indication of seizure(s). Syncopal episode lasted approx. 30 sec and Pt reports a “surreal” feeling prior to losing consciousness. Pt reports no pain of any type. Initial vital signs approx. 10 min following episode- BP 126/80, HR 77, RR 18, GLU 73.
- 36 y/o M with no PMH and never seen by a PCP p/w several years of intermittent at rest palpitations and SOB. Episodes last a few minutes and resolve. No known triggers. Pt hypertensive today 165/98. Pt has a family Hx of unknown arrhythmia in grandparent. Initial EKG as seen.
- Patient has hx of a-fib... States he had an EKG 4 days prior with his PCP who then faxed it to cardiology who said they weren't worried. This is what I had on my monitor while I got him ready for a procedure. It was confirmed he was also in bigeminy at the prior EKG. Wouldn't a cardiologist be concerned with this?
