Unrelenting rash

Patient presents for 1.5 months of a pruritic rash. Started after carrying some wood from her forested area. No one else in her home with symptoms. No systemic symptoms. She has tried steroid cream, steroids, permethrin with no relief. She has not been treated with antibiotics.

This is a 29 year old female I saw after she developed spontaneous erythema of her great toe—not to this extent. I treated her for paronychia. Since I saw her, she has seen multiple providers and has been on multiple antibiotics, topical antifungals, topical and oral steroids and had the toenail removed. She has consulted with podiatry and dermatology. Dermatology sent her back to family practice and I am unsure what to do next. This is painful, has been present for 10 months and is worse. Neither podiatry nor derm think it is fungal. Steroids didn’t change it.

45 yo Haitian F with c/o rash on UE and LE for the past 5 months. The skin is dry, rough and scaly and she described the rash as itchy. Rash is located primarily on the extremities distally but recently has migrated to the face. She was previously treated with a topical corticosteroid, oral steroids, and IM steroid. She did note some slight improvement with the treatment but symptoms returned after finishing the steroids.

Completely stumped as to what this could be on a newly admitted patient. These are her home pics as I can't use a cell phone in the patients' rooms. She has had biopsied in the past with a diagnosis of spongiotic dermatitis. It is extremely itchy, and nothing helps no matter topical steroid or topical antihistamines.
