70 yo F with freezing of gait

70 yo woman complains of gait difficulties. 5 years ago, she started to develop episodes in which she started uncontrollably walking faster and faster to the point where she was unable to stop herself. Within the next year, she developed episodes of start hesitation and often felt like her feet were stuck to the ground when trying to walk. These gait symptoms worsened and led to occasional falls. She now also experiences significant difficulty walking down stairs. On neurologic exam, she exhibits significant reduction in vertical optokinetic nystagmus, sequence effect on bilateral finger taps, increased tone in the right arm with reinforcement only, and freezing of gait on turns.
