Skin lesion

Patient Information
Female patient, 85 years old
She started with this skin lesion on the right shoulder about 2 months ago that has been growing progressively since then. She reports having used steroid creams and topical antibiotics in addition to systemic antibiotics (penicillin and ceftriaxone) without improvement. She says the lesion is itchy and painful. On examination, it presents as a plaque measuring about 20 cm in diameter with very high edges and a depressed central region and some erythematous areas.

Iniciou com lesão no ombro D há cerca de 2 meses, apresentando crescimento progressivo desde então. Refere ter feito uso de cremes com corticoide e antibióticos tópicos além de antibióticos sistêmicos (Penicilina e Ceftriaxona) sem melhora. Refere que a lesão é pruriginosa e dolorosa. Ao exame, apresenta-se como uma placa de cerca de 20 cm de diâmetro com bordas bastante elevadas e região central deprimida e com algumas áreas eritematosas.
