29 y.o. girl with a CSF leak

29 year old female presents with a headache, nausea and vomiting and drainage from the nose with positive Halo sign. Tests indicate it is cerebrospinal fluid. Patient indicates she had no trauma to the head/neck/back but she did have her cellphone hit her face while she was in bed and looking at social media. A lumbar drain was put in and my neurosurgeon wants me to drain out 10cc/hr. What could really be the cause of the increased ICP? She didn’t even break her nose when the cellphone hit her face, the phone wasn’t even 10 inches from her face when it fell and hit her.

Patient is a 35 y/o female with PMH significant for AIDS, who presented to the ER for anemia and electrolyte repletion after outpatient labs showed anemia, hyponatremia, hypokalemia. Patient found to have occult blood in stool with frequent nausea and emesis. Blood culture positive for salmonella enteritidis. During her treatment, she mentioned to a hospitalist that for 3 months she had a small soft spot on the left front of her skull. She had been seen by two other physicians she worked closely with who did not think anything of it. Appropriately, the hospitalist ordered CT head which showed a lytic lesion though with hyperdense material. Once neurosurgery was consulted, MRI was obtained finding contrast enhancement with dural thickening and invasion to the subgaleal and epidural spaces. The patient underwent craniectomy. No contrast enhancement was found on post operative MRI. No lymphoproliferative disorder was found. Cultures were positive for salmonella species. Patient is currently still receiving antibiotics from the infectious disease service with MRI scheduled 2 weeks after completion of her antibiotic course.
