Extensive case of zoster

Found this case very interesting, I work here in Brazil, state of Ceará. This 48 yo male came in referring pain and itching to the right arm one week ago. Great response to aciclovir. Will post photos after treatment.

The patient returned showing significant improvement in the herpetiform lesions, with post-inflammatory scars. Still finishing the medication regimen with acyclovir. At the moment, the patient reports pain in the third and fourth fingers of the hand, with reduced perfusion and nail detachment. Any guesses on how to do the treatment next? Thanks!
