A more anterior image shows asymmetric volume loss and bright signal involving the right hippocampus (blue arrow) and subtle enlargement of the right temporal horn (red arrow) in mesial temporal sclerosis. Etiologies include multiple seizures at an early age, febrile or intractable seizures, or status epilepticus. A low grade neoplasm with bubbly region and cortical thickening (green arrow) with features of a dysembryoplastic neuroectodermal tumor was thought to be the cause of the seizures. The patient had an amygdalohippocampectomy and lesional resection for treatment. #FridayQuizDay
