74M presents to ER via ambulance accomp. by son. Found on floor by son who states that pt admitted to son he fell and stayed on the floor X 1d. On admission, pt noted to be responsive but lethargic. No notable abrasions or lacs. VSS but hypothermic (T33.1°C) w/ euglycemic. GCS 10-11. Labs indicated multi organ system failure. During intubation, suctioned vomitus. Initiated Tazocin. Attempted to stabilize pt but became hypotensive/tachycardic in afib w/ RVR. PMHx: mechanical valve repl. (Coumadin). INR 4.5; reversed w/ beriplex. Initiated Levophed. Stat transfer to neuro institute. Great case!

Called to private residence for a DK party. 46M found lying on the floor of his living room, incontinent of urine and altered. Pt’s boyfriend states that the Pt is a chronic alcoholic and was drinking all day yesterday. Pt stopped drinking around 6pm and laid down on the floor. Boyfriend found the pt still altered on the floor in the morning. Pt has a hx of ETOH withdrawal and presents as such. Pt very tremulous, non verbal, and incontinent. Pt also had oral trauma from a suspected seizure. No other hx provided. Vitals: 168/100, HR 110, BGL WNL, O2 WNL. Besides oral trauma, no other external signs of trauma noted. Pt’s skin was cool, pale, and diaphoretic. Pt presented w/ full body tremors and tongue fasciculations. Pt also vomited during transport. Hospital started Pt on phenobarbital treatment. 3 hrs later Pt defecated on himself and wasn’t getting any better, was sent to CT scan. Thoughts?

57 Y/O F sudden severe head ache, on scene had a GCS of 10, vomited, full right sided paralysis. Couldn't get an initial BP, finally got one at the hospital BP was 236/108.

presented to ED w/severe HA, likely r/t acom #Aneurysm got and external #Ventriculostomy HH3.

40's year old motorcycle crash. Moderate speed (~40 mph), no helmet. Initial GCS 10, Pupil asymmetry, right pupil 5 mm sluggish, left pupil 2 mm sluggish. Taken to OR for right hemicraniectomy. In first picture, you can see how red the brain is (subarachnoid hemorrhage). In second picture, you can see the degree of cerebral edema by how far apart the dural edges are. There is a dural graft substitute that is underneath the native dura. Pt was extubated on POD #3, continued pupil asymmetry, mile left arm weakness. Plans to go to neurocognitive rehabilitation and cranioplasty in several weeks.
