59 y/o M presents for Stress Echo for evaluation of CP, SOB, and lightheadedness/dizziness. He recently underwent ASD closure with hx of two TIAs. Other significant risk factors include HTN, HLD, FH of heart disease in father, and occasional smokeless tobacco use. Patient achieved 85% APMHR, was asymptomatic, and had no ischemic changes at peak exercise, but had an interesting EKG during the initial exercise period and during recovery. What do you see? (first EKG is during exercise and second is during recovery)

FH Mother deceased in 70s due to CHF, brother CVA @ 61 yrs of age. Resting HR 59, BP 148/78; Peak HR 137, BP 186/72; Test terminated @ 3:17 (BRUCE protocol) due to DOE, no CP/pressure throughout test. 1st image-resting; 2nd image-just before peak exercise; 3rd image-15 sec post-exercise; 4th image-recovery. Pt presented with SOB. 1st EKG done on patients arrival, second 3 hours after the first and third was done 10 minutes after the second. Put was given 3 duonebs during her stay.
