Cavernoma cerebral
Cavernomas ou malformações cavernosas cerebrais são lesões dos vasos sanguíneos capilares cerebrais, que, ao tornarem-se dilatados e irregulares, deixam componentes do sangue “vazarem” para o tecido cerebral vizinho.
É uma doença comum (0,5% da população) mas pouco conhecida uma vez que a maioria dos portadores é assintomática. Estima-se que a reserva de pacientes portadores de lesões cavernosas cerebrais sem sintomas seja maior do que o número dos já diagnosticados.
A maioria dos pacientes sintomáticos apresentam epilepsia, cefaleia ou deficiência neurológica, usualmente em decorrência de sangramento nas lesões.
A ressonância Magnética é o método de imagem mais eficaz para detecção e análise de uma malformação cavernosa cerebral. A imagem em pipoca (pop-corn) é típica, demonstrando uma lesão heterogênea, de diferentes intensidades de sinal intralesional, que representam pequenos trombos dentro do cavernoma secundário ao seu baixo fluxo, e um halo de hipodensidade devido ao extravasamento de elementos sanguíneos, principalmente a hemossiderina.
No caso acima temos uma paciente do sexo feminino com queixa de cefaleia súbita, submetida a exame de RM do encéfalo, onde foi identificado uma imagem ovalar no lobo frontal esquerdo, com formato de pipoca, com hipersinal nas suas periferias e hiposinal central nas sequências FLAIR, T2 E T1 e hiposinal nas sequência SWI e com reace de contraste pós GD na sequência T1, compatível com cavernoma cerebral.
