A 27-year-old female patient reported constant use of alcohol and drugs at + - 1 week evolved with high-intensity headache without improvement with drug treatment.

In the Rm images of skull and angio Rm arterial we can notice several points of gliosis scattered in the skull and in the angio Rm we can notice flaps in the arterial lumen at the level of the bilateral cavernous carotid artery and marked stenosis and decrease of flow of flow of the cerebral arteries anterior and Cerebral midbrain without any change in posterior fossa arterial flow.

Based on the patient's clinic and radiological images to what conclusion can we reach the case and what better treatment for the patient and what other tests could be performed to better evaluate the case?
