A 64 yo female w hx of hypertension under medication, presenting fever, epigastric pain, loss of appetite and fatigue for a month, lab workup shows leukocytosis wbc 20500, CRP 24, blood sugar 1,50, LDH 3XN (otherwise N), chest XR and fibroscopy were normal, CT of abd demonstrating multiple lesions
What are ur thoughts? Diagnosis?

2 yo autistic male referred for possible abuse. Pallor, multiple bruises, facial swelling, lethargy. Abd mass felt on exam, left parotid swelling. WBC 5 Hgb 8.3 ply 143. Uric acid 7.7 LDH 11,882. Head CT: possible epidural hematoma, orbital masses. Skel survey: multiple submetaphyseal lytic lesions. CT shows heterogeneous mass arising from kidney. Dx metastatic #Neuroblastoma
FOLLOW UP: Patient had PCI to LAD/LCX, develop Afib amio was started, blood culture +staph Aureus, on abx, still on impella post PCI. LFT sky high, Cr increased quite a bit, Primacor was initially weaned off but had to be restarted bc low UO and low MVG. Today MVG 82, amio dcd due to LFT, resp pcr -, will likely be on CRRT by the end of the day or tommorow, still follow commands off sedation. LDH >2500, hg 8.1, haptoglobin 45, bilirubin as shown. Will update this weekend.

88 y/o F, receiving ceftriaxone and clarithromycin for pneumonia for 6 days. HF +, epistaxis with apixaban, albumin 1,7 g/dL, normocytic normochromic anemia and ascites. Large mass spotted at left lobe by thyroid USG. No cervical LAP. Calcitonin and Tg normal. Uric acid 15mg/dL, BUN 48, Creatinine 1,65 mg/dL. TBil 2, D Bil 1,4 mg/dL; AST 58, ALT 17, ALP 193, GGT 177, LDH 725 U/L. AFP 1,35, CEA 5084* ng/mL; CA 125 70,3*, CA 15-3 23, CA 19-9 2 U/mL. Any ideas?

Patient in late teens presenting after a collapse, no chest pain, no previous cardiac history, no history of sudden death in family, are there any features in this ekg that would warrant further work-up or is this just a pediatric ekg?
