74 year old white man with rash to bilateral hands "since I can remember." Areas are slightly raised, rough, and plug-like but do not seem to ever fall out. He reports callouses to the center palms of both hands. He denies chemical exposures. No recent changes, but he reports he has never been diagnosed. He was here for unrelated issues. Any thoughts?

Case 1
A 53-year-old man comes to the health unit reporting the appearance of pruritic erythemato papular rash in the palm of his hands yesterday. He reports that before there was a purulent lesion in the hand, that is in treatment with silver sulfadiazine. Has a history of Diabetes, in bad control, alcoholism, smoking and bowel tumor (diagnosed 3 years ago, it was excised, but reports hematochezia), in follow-up with gastroenterology. It reports previous hypuricemia with a similar rash on the hand. It denies other systemic signs, denies unprotected sex, denies penile lesions. Laboratory tests were requested: uric acid, serology for syphilis, HIV, and lab review. What are the diagnosis and treatment?

Hand vs saw. Pt reports with a middle finger lac complaint, we find this under the bandages. Hand surgeon consulted. Pt had great attitude and said he will be able to "Rock on" with ease now.

A 52-year-old man presented with a 20-minute history of chest pain radiating to the left shoulder and neck, associated with dyspnea and diaphoresis. He had a history of hypertension, long-term smoking, and a sedentary lifestyle. On physical examination, bibasilar crackles were noted on lung auscultation. An electrocardiogram (ECG) was performed in the primary care setting, and the patient was promptly referred to the emergency department.

A 28-year-old woman presented with a 3-day history of intense pruritus over the lower back. She reported no recent travel or exposure to new environments. She cares for a small kennel with approximately eight dogs rescued from the streets. Physical examination revealed multiple small vesicles, some clustered and others scattered, predominantly involving the lumbar region and the left gluteal area.
