Medical History: No important medical history.
Symptoms: For 3 months she has been showing sweating, tachycardia and nocturnal dyspnea. She says it’s a feeling of death.
5 days ago, pruritus appeared in the frontal region, with local erythema.

This renal profile result belongs to a 20y female underlying ESRD secondary to lupus nephritis. She was just started on dialysis for two months, then she went to faith-healer who suggested her to stop dialysis. 2 weeks after defaulted dialysis, she presented to our ED, gasping, in which emergency dialysis was commenced. She collapsed during the dialysis in which she was intubated, put on vasopressors and transferred to the regional center.
Dispatched to a free standing ED for chest pain. Upon arrival patient is found to be a 53y/o f. She states that last night she began having left sided sharp intermittent chest pain that radiated to her back and left arm. When she woke up she says the pain has become more frequent and severe. She was given 2 sublingual nitro with no relief. Additionally she was given 4mg of morphine x2 with complete relief. Patient was A/Ox4, Skin P/W/D. She appeared to be in no obvious distress, denies SOB.
