Case details:

36 years old women she suffers from redness in her face, and a little in body.

Before 7 years the condition started, she saw that her face began to be red and a little swollen as in the picture that I uploaded.

When she takes steroid or antihistamine the result was very good, but when she stops taking these medications after 7 days her face comes back to redness as in the picture.

Note: there is no itching or pain.

What is the diagnosis and what is the treatment?

She is not DM.

Not hypertension.

And doesn't have chronic disease.

No smoker.

A 58-year old woman came to my office because of rash and recurrence of skin infection of the right lower leg. Her complaints started ~20 days ago when she developed pain and redness in the anterior part of the right lower leg. She was seen by a dermatologist who diagnosed thrombophlebitis and prescribed a 7-day course of amoxicillin/clavulanate (1000 mg, bid). After 7 days, she was much better and the dermatologist prescribed only local topical therapy. Three days ago, before the current presentation at my office, she again developed pain and redness in the anterior part of the left lower leg, fever up to 38.3°C. The next day she was seen by a GP, who again started amoxicillin/clavulanate. In the same day, in the evening, she noticed rash on her extremities which was slightly pruriginous. She continued to use this antibiotic, and she improved (redness of the RLL was less intense, pain was less intense, and she didn't have fever anymore), but rash was more intense and larger. Because of this, she was referred to me. PHx was unremarkable.
PE: big papulae and noduli predominantly on extremities, with some confluent, non-tender, hard to touch areas. Right lower leg had varicose veins, painful to touch in the anterior part where redness was observed (see picture no. 4). The remainder of PE was unremarkable.
