Report case

Case for clinical and radiological evaluation.

A 36-year-old female patient arrived at the emergency room with acute headache, followed by fainting and recent memory loss. We performed a CT of the skull where an HSA was diagnosed in the occipital lobe, soon after, a skull rm was performed followed by an ANGIORM Arterial hypertension in order to diagnose some disease of the patient's cerebral vascular system, leaving the patient still in doubt of diagnosis, so the patient was referred to the hemodynamic sector where she underwent an arteriography of the intra- and extra-cranial vessels, which also showed no abnormality in relation to diseases of the cerebral vessels.

Question

Here is the question of what pathology may have caused this HSA in the patient and what clinical and radiological criteria to reach the conclusion of the possible diagnosis.

Case Follow-ups

Continuation of the HSA Case: After the patient had placed the external ventricular shunt, the patient was referred to the endovascular treatment center where a cerebral vessel arteriography was performed with the use of a 3D reconstruction to reduce the time of examination and contrast in the patient where A baby aneurysm was diagnosed in the bifurcation of the carotid artery and another ruptured bilocular aneurysm of the large neck in the right middle cerebral artery, which caused HSA.

Thus, we started the endovascular treatment with the use of a balloon microcatater to stabilize and help to place the micromoles within the large neck aneurysm with greater precision and without the risk of them entering the light of the middle cerebral artery and obstructing the same causing further damage Greater in the patient and close the aneurysm as soon as possible, thus reducing cerebral bleeding. After placing the micromoles in the ruptured aneurysm we removed the microballoon and performed a final arteriography to visualize the aneurysm closure and if we did not have any evident damage in the vessels of the middle cerebral where everything was within normality and the aneurysm was successfully treated . Diagnostic conclusion: right hemisphere HSA caused by acute middle cerebral artery aneurysm bleeding treated.

Images for radiological and anatomical evaluation of cerebral vessels.

What is the name of the anatomical variations (embryological malformations) found in the ANGIO MR images of the cerebral vessels in the images above? #angiormbrain #rmbrain

Clinical Evaluation

What are the chances of having a hemangioma on two vertebrae, the skull base, and a 1cm hemangioma on the spleen? I was reading my patients radiology report and it seems very odd to me that she has these right where she describes her pain, but they aren’t usually associated with pain. Any thoughts?

Case Summary

A 27-year-old female patient reported constant use of alcohol and drugs at + - 1 week evolved with high-intensity headache without improvement with drug treatment. In the Rm images of skull and angio Rm arterial we can notice several points of gliosis scattered in the skull and in the angio Rm we can notice flaps in the arterial lumen at the level of the bilateral cavernous carotid artery and marked stenosis and decrease of flow of flow of the cerebral arteries anterior and midbrain without any change in posterior fossa arterial flow. Based on the patient’s clinical and radiological images to what conclusion can we reach the case and what better treatment for the patient and what other tests could be performed to better evaluate the case?
