Ultrasound images in an infant with a septate uterus (arrow) | Figure 1

Cincinnati Children's Radiology

Ultrasound images in an infant with a septate uterus (arrow) and imperforate hymen show hydronephrosis of the left kidney (image 1), and fluid with layering debris within the right and left lumen of the vagina. A septate uterus is the most common Mullerian duct anomaly. Patients often present later in life after multiple miscarriages. An imperforate hymen can lead to hydrometrocolpos in the neonatal period (due to maternal stimulation) or later in life after menarche. Infants with hydrometrocolpos may present with urinary obstruction related to external compression of the ureter by the dilated uterus. #TummyTuesday #AbdomenTuesday

AP (image 1) and crosstable lateral (image 2) of the abdomen in a neonate born prematurely shows extensive pneumatosis (arrows) and portal venous gas (arrowhead). Necrotizing enterocolitis is the most common cause of pneumatosis in neonates. It typically occurs within the second or third week of life in prematurely born, formula-fed infants. Lateral radiographs are essential to evaluate for intraperitoneal free air as this changes the care from medical to surgical. #TummyTuesday #AbdomenTuesday](https://app.figure1.com/case-detail/10b58ec3-64ee-43f2-8cab-2c7aaaf057ec)

Longitudinal ultrasound in a girl with an imperforate hymen show blood products filling and distending the vagina (arrows). The distended vagina is displacing the more anterior urinary bladder. Blood extends into the uterine cavity. Note that the uterus (arrowhead) is much smaller than the distended vagina. Imperforate hymen is a congenital abnormality. Patients may present in either the neonatal period with a distended abdomen or signs of urinary obstruction or as an adolescent with primary amenorrhea.](https://app.figure1.com/case-detail/194a0ef3-4ff6-412c-87d2-29288119999b)

Longitudinal ultrasound of the uterus in a girl with primary amenorrhea shows a dilated vagina (arrow) filled with hypoechoic material and a dilated uterine cavity (dashed arrow), with the cervix (arrowhead) visible. Hematometrocolpos refers to the distension of the uterus and vagina with blood. In adolescents, it is most commonly caused by an imperforate hymen or transverse vaginal septum.
