this 4 year old starts out with a small red spot on hand in within 2 weeks it looks like the last picture. Does not itch. No drainage. Mostly flat. ALL labs within normal limits. Dermatologist was unsure. ID doc was unsure. Anyone have an idea? It's very strange...kid lives in Texas.

40-year-old healthy male presents to emergency department with "insect bites". Pt is traveling in a recently-purchased used RV with his family in the Western US. States the lesions are itchy. They are concentrated mostly on his upper arms and hands, also sparsely on his lower limbs and trunk. Worst lesion is ~1.5cm diameter to right thumb with erosion and eschar. Complains of chills today, but no fever. Vital signs are within normal limits in the ED. Any thoughts?
update on 4 year old. Wounds are healing by using a medical honey scrub which seems to be working well. Child has had a negative blood culture for 4 days. Would anthrax show up on a C/S from the wound? The biopsy and wound culture were negative . The child is currently still taking Cephalexin for redness around the wound. I strongly doubt this is a brown recluse bite because he's a very talkative 4 year old that I would assume would've felt 2 bites and would've had symptoms. He's had zero pain or itching. So does anyone else have an idea what it could this be?
the patient came back from a trip in the jungle in central america with a little bump, looked like a muscito bite without any pain or itch. Some weeks later it looks like this. Same but smaller on his finger, same hand. Biopt, next week the result. What do you think?
This 4-year-old from Montana has had this progressive lesion on his right arm for 4 months. Describes it as itchy but sometimes painful. No known bite nor other medical problems; treated with topical steroids without benefit. I would have leaned towards a fungal infection, especially given its progression on steroids, but the violaceous discolaration has me concerned. Any recommendations on possible diagnoses or recommended treatment?
