Skin rash in a 41 yr old male
A man has been admitted to the medical ward for a chest infection. Upon physical exam, he was found to have this rash in form of light brown macules and patches involving mainly the trunk (neck, chest, abdomen, back) and upper portion of the arms, sparing other parts of his extremities including palms and soles, as well as the face. The rash has been present as long as he remembers since childhood and it is otherwise asymptomatic. He reports that it gets darker at some periods of time, then fades, but never disappears. Upon further inquiries, he reported a similar rash in his paternal aunts and his brothers, but none of his sisters.
What are the likely diagnosis and differentials for this rash?

\ A 34 year old male patient presents with the progressive appearance of a dark lesion on his lower lip, which he reports has been growing over the years. Initially, he noticed a small, barely noticeable spot that gradually increased in size and is now slightly elevated. He denies symptoms such as itching, pain, bleeding, or ulceration. The patient describes a small “bump” that can be felt upon touch and mentions a desire to pop it, though he has not attempted to do so. He denies any history of local trauma, lip biting, or prior manipulation.

\ Late 20s male told me that he had "some kind of shingles outbreak". Upon observation I thought pityriasis rosea but no typical herald patch and it is quite pruritic. He reports the lesions continue down his legs and have been present for almost a month. Related Hx: He had abnormal blood work during a check up several months ago so his MD ran additional testing and his ANA was negative. He had reported having rashes before and they recommended tanning which he said helped but he's never had a rash like this and didn't have it when they were doing his additional labs. He'll be going to a dermatologist but I want to see what you guys thought. Syphilis would have show up on blood work right? And this rash appears more papular. Is this likely related to his other symptoms or an unrelated outbreak? In picture 5 he reports that those lesions developed from him itching.
