Sagittal MRI of the pelvis in a neonate shows a giant mass extending off the inferior aspect of the image. The mass is heterogeneous with cystic and solid components and containing large vessels. Sacrococcygeal teratomas are classified by the amount of internal tumor. Stage I tumors are completely external while stage IV tumors are completely external. #TummyTuesday

26yof presented for EUS for local staging of rectal cancer. Flexible sigmoidoscopy showed 2-3cm lesion in the rectum approx 8cm from anal verge. No lymph node invasion. Lower US: mass was hypoechoic and heterogeneous 9.9 x 31.8mm. Invades muscularis propria. Stage 1, T2. Internal and external sphincters were intact.
Bivalved kidney showing a textbook appearance of Renal Cell Carcinoma. The orange-yellow color is typical and not representative of necrosis. At the tip of the blue arrow, you can see the mass appears encapsulated and confined to the renal cortex. The mass abuts but does not involve the renal sinus fat or the renal pelvis. Invasion of renal sinus fat, renal pelvis, or perinephric fat will change the patient’s stage. For kidney cancers, the important measurements to keep in mind are 4cm, 7cm, and 10cm. A 4cm mass is T1a, between 4cm and 7cm and you’re at T1b, over 7cm but under 10cm and you’re T2a, and once the tumor is more than 10cm in size, it becomes a T2b. All T1-2 stages are for when the tumor is confined to the kidney and not invading the Renal Vein.
#Uterus displaying #Adenocarcinoma. Arrow points to the more solid aspect of tumor. Mass appears to occupy entire anterior and posterior endometrial canals. Even without cutting, looking at how thin the myometrium (muscle) is, I can say that this tumor is invasive. The stage of these tumors is determined by a number of factors, including whether the tumor invades less or more than one half of the thickness of the muscle. The stage will be even higher if the tumor invades completely through the myometrium and presents itself on the uterine side (serosa).
Axial T2-weighted MRI in a child with hepatoblastoma shows PRETEXT IV multifocal tumor (arrowheads) and extensive portal venous thrombosis (arrow). Hepatoblastoma is staged using the PRETEXT staging system. The PRETEXT number is determined by identifying the number of contiguous hepatic sections involved by tumor that would have to be resected in order to completely excise the tumor.
