Case Description

30' Male with painless rash/nodules growing on the face since 3 weeks. Patient under Mopral. Has cutted wood and used a soap with Linalool the day before. D1: 6 to 8 big nodules have appeared at the same time on the face - D8: some of them have disappeared but others have appeared so we've started Fucidine, without effects - D10: Dermatologist gave him Aerius and Locoid but it got worse (more little red painless rash growing) and little red rash on the neck and arms appeared also - D18: there are more red rash and nodules. Patient has stopped Locoid. No rash on the rest of the body. Diag?


Additional Cases

Patient presented to pharmacy with attached photos requesting hydrocortisone for sun reaction. Further questioning indicated a recent hx of moderate sunburn followed by appearance of red pruritic rash focalized in area of worst burn. The rash also causes intense pain/irritation when scratched or rubbed. Rash appeared approximately 4-5 days following sun exposure. Had used aloe on burn up to this point. Patient indicated pmhx of mild heat rash following sun exposure but also stated this feels much more uncomfortable than any prior heat rash and time to onset seems to indicate a different etiology. Provided patient with 1x tube of 0.5% hydrocortisone to try, however I want to make sure I’m not missing something that should be referred to an MD. Patient was told to return in 3 days if HC didn’t seem to be helping, I’m hoping I’ll have a bit more info to give at that point.

This previously healthy 11yo boy presented with a faintly itchy circular rash with scale on his neck five days ago. Over the last two days these additional oval annular lesions have appeared all over his trunk. He has no fever or constitutional symptoms. He says that the rash is faintly itchy and sometimes it burns a little bit, but that it is not very severe. Do you recognize this?

52 y/o female presents with skin rash that appears bumpy and red/pink in color. Rash appeared about 1 day ago. Patient comes with complaints that the rash is itchy, unbearable. Patient does not have any specific allergies. The patient explains that they have been under excessive stress as well.

30yo female patient, NKA or PMHx, not on any medications. Rash develops on anterior leg, beginning as small red vesicle and progressing into larger vesicle before draining yellow serious fluid. Progresses into larger, dry, red patch. Occasionally pruritic. Has been treated with antifungals, antibiotics, and topical hydrocortisone. In first photo, new spot is beginning superior to the original lesion. Pt has been without fevers the entire time. Any help or advice on what this rash could be will be useful.

A 52-year-old man presented with a 20-minute history of chest pain radiating to the left shoulder and neck, associated with dyspnea and diaphoresis. He had a history of hypertension, long-term smoking, and a sedentary lifestyle. On physical examination, bibasilar crackles were noted on lung auscultation. An electrocardiogram (ECG) was performed in the primary care setting, and the patient was promptly referred to the emergency department.

A 28-year-old woman presented with a 3-day history of intense pruritus over the lower back. She reported no recent travel or exposure to new environments. She cares for a small kennel with approximately eight dogs rescued from the streets. Physical examination revealed multiple small vesicles, some clustered and others scattered, predominantly involving the lumbar region and the left gluteal area.

A 4-month-old male infant presented with skin lesions localized to the chin for the past 3 days. Physical examination revealed multiple small pustules with surrounding inflammatory signs on the chin, along with a few scattered papules on the chest.

Patient in late teens presenting after a collapse, no chest pain, no previous cardiac history, no history of sudden death in family; are there any features in this ECG that would warrant further work-up or is this just a pediatric ECG?
