Acute pain with shock

A 62 year old male patient, presented to ER complaining of continuous severe intrascapular pain for 4 hour duration, radiated to chest, increased with movement and supine position severely in nature 10/10 associated with sweating and general fatigue, no history of nausea or vomiting.

Vital signs measured and showed, BP:77/45, HR :40, SAT: 92, Random blood sugar: 136.

ECG was done and showed bradycardia.

So give him one ampule of atropine... after 10 min his BP 125/80, ECG repeated again and showed 1mm ST elevation on lead 2, V1 and V3, so give him 10 mg morphine and admitted him to CCU.

AORTIC CT ANGIO was done and showed 5cm aneurysm on the ascending aorta, another thing was normal.

Cardiac catheterization was done and showed patent stent on LAD and Circumflex, and 50% stenosis on RCA.

Patient kept under observation for 24 hours then discharged to home.
