Metastatic Lung Cancer with intra and supratentorial lesions
60s yo female adm for mechanical fall w headache. 2 months previously dx and treated for lung cancer. Deconjugate gaze, and poor cerebellar function, dysarthric. Multiple supra and infratentorial sub centimeter lesions. Larges lesion was right cerebellar w mass effect on 4th ventricle. Extensive conversation w pt and family, they requested surgery. Path c/w metastatic small cell carcinoma. Post op cerebellar function improved, and participated well w therapy. Ultimately passed 3 weeks after resection due to pulmonary deterioration and family desire for hospice.

Coronal T1-weighted image (image 1) shows a meningioma (arrow) at the right cerebellar pontine angle. After contrast was administered (image 2), there is diffuse enhancement of the lesion with minimal thickening and irregularity of the adjacent petro clival ligament and lateral margin of the right tentorium. Enhancing cerebellar pontine angle masses typically represent either a vestibular schwannoma or a meningioma. While this is the most common location for a meningioma in the posterior fossa, vestibular schwannomas are more common. Differentiation of the lesions can be difficult. The presence of a dural tail may help to confirm a diagnosis of meningioma.
84yoM w/complaint of HA/slurred speech. h/o severe dementia - no family living in area. R cerebellar hemisphere ICH measuring 2.5 x 1.6 cm. ICH score of 2.
