Patient was originally treated under primitive conditions in Madagascar (without cultures available), with daily wound care and 10 days of Amoxi/Clavu. When arrived in home country culture was positive for S. aureus resistant to penicillin and erythromycin but sensitive to Dicloxacillin. 10 day course given + weekly wound care. Spider was never caught but several other people were bit (one was bit 7 times) at the same geographical location and with similar symptoms - developing pain, local necrosis with eschar, no systemic symptoms and not always infection. Therefore still suspected sac spider bite. Can post pictures from these bites as well if there's interest.
