Case Discussion
60 year old diabetic patient. Presented with non-painful, pustular skin lesions localized to flexor surface of forearm (picture taken after patient's PCP drained lesions). Patient states "this is the first time" for him and reports no associated symptoms. Patient is afebrile and physical exam is unremarkable. What are some possible differentials? Thank you.

53 yo male HIV positive with CD4 cell count <50 cells/ul who started HAART in two months before this lesion appeared. One month later a verrucous , crusted lesion with central ulceration developed in the elbow. Patient comes from the jungle. HAART: TDF/3TV/EFV. A giemsa stain was positive for leishmania sp. Patient was started on daily IV sodium stibogluconate. At 6th day of treatment lesion seems to get better. Is it possible for this to be a case of cutaneous leishmaniasis IRIS (uncover)?
