911 for male complain of chest pain. Upon arrival 59yo/m found sitting on couch. He states that he woke up around 8:30am with N&V, dizziness and SOB. Patient presents A/Ox4, skin PWD leaning back gripping his chest saying "it feels like my heart is beating out of my chest". He states he is a dialysis patient and this happens when they "take to much off". He is showing rhythm above. Patient refused IV and meds during transport.

Patient has hx of a-fib... States he had an EKG 4 days prior with his PCP who then faxed it to cardiology who said they weren't worried. This is what I had on my monitor while I got him ready for a procedure. It was confirmed he was also in bigeminy at the prior EKG. 50yM Chinese guy ( recently immigrated US) with no PMH came to the ED after 5 episodes of syncope in the past 4 days. Patient states that he fell and woke up with his fist clenched and in a pool of urine. As per sister, she states that he saw him on the floor with eyes rolled back and muscle stiffness. Patient also says that he had chest tightness and palpitations before the last syncope episode. Patient runs 22-40km everyday and he also states that he puts a lot of salt in his food.
