update. 4 punch biopsies later, (one neg direct Immunofluorescence), all consistent with intra epidermal blisters, spongiotic derm with eos & lymphs (no neutrophils), mainly superficial. Ddx includes 1) rare/genetic blistering dermatosis, 2) atypical bullous pemphigoid/PV, 3) arthropod bite or drug eruption. All former listed ruled out in favor of drug rxn. Pt disclosed thyroid meds were switched (different brand and dose) prior to eruption. Meds switched - rash apparently resolving with PIH. Thx jplonza and nebushwalker!

\n little blisters erupted all at once both hands with swelling. Looks like pompholyx but the eruption was sudden, exactly the same both hands and in a patient with not dermatitis/eczema/atopy history. I'm treating as pompholyx but found the onset unusual compared to what I usually see. Patient is well.
\n 50 YOF admitted to psychiatric facility. Recently hospitalized at another facility. Prior drug use, denies IV drug use. Denies any injury, exposure, burn. I started keflex 500 mg QID and gave 1 gram rocephin IM and applying clotrimazole topical. Today it looks worse. Switching to Bactrim and keeping Kelfex BID. This is a psychiatric Hospital and I have no access to IV meds. Any insight to this infection would be appreciated if anyone has seen anything like it before. The white areas aren’t blisters, and the top of the white patches are waxy. Other ear looks normal. Vitals normal. Pt reports only mild discomfort.
