22 year old female presents to the emergency department with complaint of vaginal bleeding. She was due for her period 3 weeks ago but it never came. This morning she noticed some spotting, the passing of a few clots, and some lower abdominal cramping. There has been no other vaginal discharge, no other abdominal pain. What is your differential? What does the ultrasound show? What additional testing is needed? Is she safe for discharge home?

A previously presented case came to me at rural area for the same complaint who she had before two months, which was milky watery vaginal discharge but this time she described her the amount like a menstrual cycle of watery discharge, which associated with lower back pain and Dysuria. She has to change pads everyday due to the discharge. Its odorless and non-sticky. She took Bactrim for 1 week and Metronidazole for 14 days it eased her condition previous month but after ending her cycle this month the condition became worse. Blood investigations and vaginal swab culture showed no growth and no abnormality. What do you think the diagnoses is and what are next steps to end the discharge?

30 years old female presented with vaginal discharge for 4 months! The discharge is watery - milky odorless discharge during the whole month associated with dysuria, mild to moderate burning sensation during sexual intercourse, unilateral left sided suprapubic pain and back pain same side, no vaginal itching or edema or redness, no fever, no vomiting. The gynocologist prescribed these tx during these 4 months fluconazole, ectrose, clotrimazole, miconazole, sodium bicarbonate vaginal duoch, metronidazole. Once she discontinued the antifungal tx the discharge appears. Single sexual partner, no contraceptive used. Investigations were unremarkable, pt -ve. CBC normal, abdominal us normal, vaginal swab culture was negative, GUE showed UTI and received tx with nitrofurantoin. Now the gynecologists wants to start with trimethoprim for one month, she came to me worrying about not being pregnant despite trying for 5 months to be pregnant. What's the most likely Dx?
