30 y.o. presents for itchy, solidarity vesicle/papule lesions that first presented on anterior shaft of the penis, followed by the hand and axilla as shown. Pt had no lympadenopathy, fever or other symptoms. No recent travel or occupation exposure.
Pt had limited pmhx but reported recent B/l ear infection for which he took a full course of amoxicillin. Pt denied recent sexual activity, high risk behaviors etc and had never been diagnosed w an STI. No substance use or exposure. Pt reported chronic GERD but was otherwise healthy and took no medications. NKA.
24 y/o f with rash(no itch) and nausea. No fever, no GI issues. Treated as allergic reaction with 50mg Benadryl and 4mg Zofran. At ED pt was given prednisone and epi. Pt discharged and rash was improving. Possible diagnosis at discharge was possible scombroid fish poisoning. Pt discharged and upon arrival to home, the rash reappeared in a different location on body however more severe. Pt was transported to hospital without treatment due to recency of medication administration. I have attached a photo of the urticaria. I almost looks like blotches of water paint. Any ideas on diagnosis and the rash? Not your typical rash. Any help would be appreciated.
Pt with weeks of pain burning and discomfort, lesions in various phases, weeping and bleeding. Pt is on prednisone and antibiotics but we’re stumped. Thanks!!
