Case Discussion

Recurrent subarachnoid hemorrhage with paraparesis. Cerebral MRI normal. We have only 0.4 T. Can it be dural fistula? How can we rule it out?

28 yo male with T10 Spinal cord injury from motor cycle accident 6 years ago. He depends on a transportation service get to appointments, but it is unreliable and often is late, which is frustrating to him and makes it difficult to go out or even want to go out of the house. He has a home health nurse that comes to his house daily to assist with his care. Lately he has been feeling increasingly sad, he misses doing the things he used to do like sports and engaging with others in community.
PMH: spasticity, neuropathic pain, dyslipidemia, pre-diabetes, pressure wounds, depression, urinary incontinence, constipation
Medications: pregabalin, baclofen, atorvastatin, duloxetine, oxybutynin, miralax
Social History: Currently unemployed. Prior to his injury he loved playing sports, especially surfing, and had a community of friends through sports. He said one day he tried to go to the beach, but his wheelchair can’t go on the sand. He feels like he doesn’t get to enjoy the things he used to and that makes him sad.
Any thoughts on how we can help support this patient to enjoy the best quality of life for him? How can we meet his specific needs? Is there a role for telemedicine in this case?

22F presented with complaints of back pain and lower limb weakness. Preop imaging demonstrated dorsal (thoracic) spine instability with D8 (T8) collapse and cord compression. We performed anterior thoracotomy and D8 (T8) corpectomy with an expandable cage reconstruction and stabilisation. Can you guess the pathological diagnosis? I have answered in the comments. #apollohospitals

motorcycle collision. during ICU stay required tracheostomy and feeding tube - clinically appeared like a C4-5 level injury.

Sagittal T2 MRI of the upper (image 1) and lower (image 2) spine show a large holocord syrinx (arrowhead) caused by a Chiari I malformation (arrow). Chiari I malformation is characterized by inferior migration of the cerebellar tonsils through the foramen magnum. When a Chiari I malformation is present, the cerebellar tonsils can be pointed having a peg shape. A syrinx occurs with greater degrees of compression. Typically, if a syrinx is present, it is small and limited to a short segment of the spinal cord.
