Case details
70 y/o male, c/o weakness x 6 hours. Hx HTN, DM, high cholesterol. Normotensive. BGL 353 mg/dL. CO2 25 mm Hg. RR 16, SpO2 96% on RA. HR 30 w/ peeked T waves, suggestive of hyperkalemia. Family states last time this happened, pt. was dialyzed in ER & required transcutaneous pacing. FD administered Zofran and 0.5 mg Atropine w/o change, IV infiltrated & site showed extravisation. Pacing pads placed as precaution. Pt. transported w/o incident. IV access re-established, blood drawn for POC lactate and K+ level, which was 6.8! Pt. tx in ER w/ success.
