Lisfranc's fracture
Dislocation diabetic who was unaware of injury.. Any thoughts?
More about this case
This is a follow up on my earlier case - fracture/dislocation of humerus neck. The patient is in much pain not just because of the fracture, but also dislocation. It seems reasonable to want to put the joint back in a better position. As such with strong analgesia and adequate sedation reduction is attempted. With all the muscles relaxed, the humeral head goes back into position without vigorous manipulation. Once reduced, you are left with only one problem - a fractured neck of humerus. A CT scan and surgical intervention may well be required.
37 y/o male presented to the ED BIBA with complaint of L wrist pain, riding motorcycle someone stopped short in front of him, he stopped short and went over handlebars. No other injuries, transferred to medical center with hand specialty evaluations.
Ankle fracture/dislocation. You know the patient will be on your ORIF list for the day. Should you really obsess about how good your post-reduction film will be? It's a dynamic fracture after all, fragments could 'pop' out of position with minimal movement.
Sunrise and PA views of the knee show lateral dislocation of the patella. The dislocation is associated with a displaced osteochondral fracture of the posterior aspect of the patella. Patellar dislocations typically occur due to a shallow femoral trochlea, this case occurred due to direct trauma. Osteochondral fractures occur in 25% of traumatic patellar dislocations.
Trending now
A 52-year-old man presented with a 20-minute history of chest pain radiating to the left shoulder and neck, associated with dyspnea and diaphoresis. He had a history of hypertension, long-term smoking, and a sedentary lifestyle. On physical examination, bibasilar crackles were noted on lung auscultation. An electrocardiogram (ECG) was performed in the primary care setting, and the patient was promptly referred to the emergency department.
A 28-year-old woman presented with a 3-day history of intense pruritus over the lower back. She reported no recent travel or exposure to new environments. She cares for a small kennel with approximately eight dogs rescued from the streets. Physical examination revealed multiple small vesicles, some clustered and others scattered, predominantly involving the lumbar region and the left gluteal area.
A 4-month-old male infant presented with skin lesions localized to the chin for the past 3 days. Physical examination revealed multiple small pustules with surrounding inflammatory signs on the chin, along with a few scattered papules on the chest.
Patient in late teens presenting after a collapse, no chest pain, no previous cardiac history, no history of sudden death in family, are there any features in this EKG that would warrant further work-up or is this just a pediatric EKG?
