Term newborn with immediate respiratory distress following prolonged ROM and maternal chorio, initial APGAR 1/2/2. Home birth, Intubated twice in field with positive pressure ventilation. #1 image initial CXR, by this time was only on 2L NC. #2 is later in the day, #3 is the following day. NO TREATMENT except supplemental O2 and abx for chorio; no needle decompression given. Transferred out of NICU out of 2 days to floor, doing well. (Dx: large L PTX with mediastinal shift and pneumomediastinum, spontaneous resolution).
