Case Overview
59 y/o male with sharp and pressure like pain in the center of his chest, non-radiating, 5/10. Pt has HLD but only takes OTC supplements to control HLD. Pt has family hx of MI. Father and 2 brothers had MIs and died.
All vitals were stable and his pain got better to a 3/10 at the hospital.
Question is, is this Wellens Syndrome? He was still having pain and the deeply inverted Twaves are not in V2 or V3.

73 y/o male pt brought in by EMS presenting with 8/10 chest pain exhibited by Levine's sign, radiating to L arm. Pain began at rest and was unrelieved with 324mg ASA or 0.4 SL Nitro. Pt was diaphoretic and also complained of shortness of breath. Pt has had two previous MI's and states this feels like his last one. VS all w/in normal limits. Pt given O2, IV fluid and 100 mcg of fentanyl en rout to ED. 12 lead showed ST elevation in V1-V3. ED diagnosis: A-fib w/ early repolarization. Pt placed on cardizem drip and discharged the next day.
C/t 47 yo male c/o sudden onset left sided chest pain with radiation to L jaw. Pt describes pain as mild pinching sensation in chest and moderate ache to L jaw. Pain score 4/10. Pt had similar episode 2/7 which self resolved after 20 mins. This time pain not resolving. Pt appears comfortable, nil diaphoresis, Nil PMH, non smoker, occasional ETOH, strong family hx of CAD: Father has had x3 MI's, uncle died of MI at 37, Grand father died at 62 from MI. All obs WNL. 12-lead as above. Trt: 300mg Aspirin, 10mg Morph (GTN withheld) + rapid transport to catch lab. Pt had 100% occlusion of circumflex artery. x1 stent placed. Pt doing well.
71 yo F with C/C of chest pain x1 hr. Pt has past Mx of PEA cardiac arrest, inferior MI, CAD, HTN. Pt denies nausea, vomiting, shortness of breath. Vitals are 128/67, HR 82, RR 18, SpaO2 98%. I see a lateral anterior MI w/ possible STEMI.
77 y/o male complaining of chest pain that started 20 min prior to ems arrival. Pt took 3 of his own nitro tabs did not alleviate symptoms. Pt has extensive cardiac history receiving doc. Possible dx was a non-stemi MI.

Patient in late teens presenting after a collapse, no chest pain, no previous cardiac history, no history of sudden death in family. Are there any features in this EKG that would warrant further work-up, or is this just a pediatric EKG?
