Summary: Thanks for reviewing this otherwise healthy male in his early 30's. Fractured right posteromedial tibial plateau while running in 2023. There was delay to diagnosis due to initial XR’s being read as normal- was never non-weight bearing. The rehab has been up and down, but felt significantly better this summer/early fall. As activity ramped up, the pain returned.
Investigations: MRI shows improvement of fracture. Ordered a SPECT-CT that showed increased metabolic activity suggestive of ongoing perfusion/healing. Now on crutches for several weeks and symptomatic with any weight-bearing 15 months post-injury.
Treatments: - Calcium po 1200mg/d
- Vit D po 11,000 IU/d
- BPC-157 sc 300mcg BID
- TB-500 sc 1mg/d
- MK-677 po 12.5mg/d
- Collagen and gelatin po daily
- Zoledronic acid 4mg IV
- Red light therapy three times weekly
- Exogen LPUS ultrasound treatments daily for 8mo
- Physiotherapy
- Medial unloader brace
- DEXA bone density scan - 99%
- Endocrinology work-up (normal)
- 6 XRs, 3 MRIs
- PRP platelet lysate
- Prolotherapy
