A 31 year old Pakistani gentleman presented with severe colicky umbilical pain, N/V and constipation of two days duration. An abdominal x-ray was taken, and an NGT was inserted. Pretty straightforward right? Intestinal obstruction (SBO). His abdomen, however, was virgin and his hernia orifices were all intact so a CT was done and not even that could reveal the cause of the obstruction! so he was sent for surgical exploration where the moment they pulled out the bowel loops, they could identify the cause. Throw some guesses!
