6-8 mm nodular rash

60 yo M. Lives in care facility. C/O 2/52 of nodular rash first appearing on right hand with a red nodular lesion that then broke down to the appearance in the image shown of the hand. Then the rash spread to the right elbow and then to the left elbow, close up image is of one approx. 8mm lesion at the right elbow. This lesion had 2 smaller lesions developing on the medial aspect of the elbow and one other to the lateral side. The lesions are hard, there is no discharge, there is a scale overlying them, they are mobile, they are not itchy, non-tender and are not fluctuant. The lesions have subsequently spread to the left ear. Ddx: ?herpetic whitlow, ?nodular Perrigo, ? Nodular scabies. I'm lost as to a diagnosis. I have him flucloxacillin in case it's bacterial but to be honest I doubt that. We're in an area with a lot of scabies. Any help would be much appreciated. Cannot recall medications.
