A 11 y/o patient with presents with dyspnea and palpitation. Which one of the following would be your first step to elucidate this ECG? This ECG shows ST-T wave changes in inferno-lateral leads and fragmented QRS (even in 40 Hz electrocardiogram) localized to V2 and V3. Echocardiography showed a mass localized in right ventricular free wall. Wide spectrum of findings can present in ECG of patients with cardiac tumors. Atrial and ventricular arrhythmias, conduction blocks, nonspecific intraventricular conduction defects, fragmented QRS complexes, nonspesific ST-T changes, low QRS voltages, left or right atrial abnormalities, ST elevations and even incessant ventricular tachycardia. However, the mainstay of diagnosis is echocardiography. #cardiopaedia
I have been searching for S1Q3T3 since I learned about it, and I think I may have found it! This lady is 1 day post knee replacement, and admits to being very immobile at home the month prior due to pain. This was her ECG right after a syncope episode at the hospital during PT. CT showed bilateral scattered emboli. Am I reaching for straws? I definitely see the inverted T in Lead 3 and a slight Q wave. It’s hard with S1 because it’s superimposed on ST depression. Thoughts?!
