51 y/o Male who states he "popped a zit" on his scrotum. Within 4 hours, he swelled up and presented to ED. They gave him an ABX script and sent him home. Woke up the next day and scrotum, peri area, inner thighs and rectum had started to turn black. Presented to a small, rural ED and they immediately transferred him to us. Patient was taken to emergent surgery. Diagnosis of Necrotizing Fascitits vs. Fourniers Gangrene - surgeons removed 40 square cms in the first surgery. This was post-op Day 1. To complicate things further, patient was severely septic, on 3 pressors and in full blown ARDS... It's been 3 months, still in the hospital, 4 surgeries, obviously has a colostomy, and the graft process hasn't even started.
