Case Description
24 y/o f with rash(no itch) and nausea. No fever, no GI issues. Treated as allergic reaction with 50mg Benadryl and 4mg Zofran. At ED pt was given prednisone and epi. Pt discharged and rash was improving. Possible diagnosis at discharge was possible scombroid fish poisoning. Pt discharged and upon arrival to home, the rash reappeared in a different location on body however more severe. Pt was transported to hospital without treatment due to recency of medication administration. I have attached a photo of the urticaria. It almost looks like blotches of water paint. Any ideas on diagnosis and the rash? Not your typical rash. Any help would be appreciated.

27 YO F who presents with symptoms of discoloration of right second/third digit and left third digit who saw podiatry which recommended vascular referral for vasculitis. Also reports weight loss of 22 pounds in 3 months, nausea, blurred vision with bilateral eye socket pain, increased heart palpitations, clear nasal discharge for 3 months, joint pain, hair thinning. PMH of POTS, endometriosis, HEDS, nutcracker, pelvic congestion, migraines. ECG shows NSR, increased right atrial enlargement, rightward axis, ST depression in anterior leads. CT of head and abdomen negative for aneurysm. Any other possible DX for patient/management for symptoms?
60 yo M. Lives in care facility. C/O 2/52 of nodular rash first appearing on right hand with a red nodular lesion that then broke down to the appearance in the image shown of the hand. Then the rash spread to the right elbow and then to the left elbow, close up image is of one approx. 8mm lesion at the right elbow. This lesion had 2 smaller lesions developing on the medial aspect of the elbow and one other to the lateral side. The lesions are hard, there is no discharge, there is a scale overlying them, they are mobile, they are not itchy, non-tender and are not fluctuant. The lesions have subsequently spread to the left ear. Ddx: ?herpetic whitlow, ?nodular Perrigo, ? Nodular scabies. I'm lost as to a diagnosis. I have him flucloxacillin in case it's bacterial but to be honest I doubt that . We're in an area with a lot of scabies. Any help would be much appreciated. Cannot recall medications.
Patient's condition started on lower right flank and spread to thighs and left lower flank within a week's time. Patient endorses pruritis unrelieved by Benadryl creme and cortisone 1% OTC creme. Rash is spreading by the day and no pus or discharge is present. Folliculitis has been introduced as a possible diagnosis. Any thoughts?
Pt came in with allergic reaction to possibly shrimp. Started on prednisone, Zantac and continue with Benadryl for 2 days. Improvement after 2nd day and was rash free for approx half a day, now presents with rash in last picture. Any thoughts?
