Update to 88M with generalized weakness. Machine read pt's EKG as Afib, though the regularity of the R-R intervals suggested a junctional vs sinus rhythm. A Lewis lead tracing confirmed P waves were present. The Lewis lead, aka the S5 lead, is a configuration that maximizes P waves relative to the QRS, as the limb leads are configured to run parallel to the vector of atrial depolarization, and perpendicular to that of ventricular depolarization. It is useful for detecting P wave dissociation in VT-vs-SVT+aberrancy, and in the setting of AV blocks.
