30yo/m came to er for palpitations otherwise asymptomatic. received 6&12 adenosine in er no change. Pt came to floor on cardizem gtt hr 186 bp 105/80. Card d/c'ed gave 5mg lopressor iv push no change in hr. Bp dropped 82/57. Amio started. Bp dropped to 44/36 pt lost consciousness. 1 shock delivered pt converted to nsr bp 90/70 sent to icu. What is this rhythm?

25 y/o patient to ER for palpitations. HR 220's, 6mg Adenosine given with no change, EKG shows conversion after additional 12 mg Adenosine given. Pt alert and awake the whole time
56 y/o in for appendicitis but on cardiac monitor for "heart issues". SVT. Highest was 230 bpm. Pt was completely asymptomatic. Tried vagal maneuvers, gave 2 rounds of adenosine which dropped the hr, but came right back up. Gave IV bolus 10mg Cardizem and 5mg drip. Dropped and again came right back up. Pt to ICU for sedate/shock. Great learning experience for a me.
sinus #Tachycardia in otherwise healthy 21 YO female sudden onset while sitting down, lasted for 4 hours before HR dropped below 100bpm blood pressure was 169/120 when presented to ER, also came down slowly with HR. Patient experienced HR of 232 a week ago when running, and HR of 49 yesterday while laying down at home. Patient has kept a consistent track of HR over the past month after noticing #Palpitations and chest tightness. What tests would you perform next? Why sudden onset?
