May I share case outside dentistry?
A male patient, 26 yr old come to our dental clinic to discuss his problem. First of all, we are in deep palm plantation in the village and there is no medical doctor near our clinic.
He said that his penis has a red spot. He is not feeling pain or anything, just a red spot. He never had a sexual relationship with anyone. This red spot appeared since 1 month ago.
Do you have any idea what this is? And treatment for him? Thank you for your help.

Patient came with these small bump like blisters on his back and a little bit on his arm. Patient stated that it is very itchy and that he went to a barbecue in the forest with a couple of his friends and then he noticed afterward he got these spots. What do you think is the possible diagnostics for this?
Case Detail
This patient has been diagnosed since his childhood with dermatitis. Most of the time during the past 12 years he was on corticosteroid creams. Last 1.5 years his symptoms have worsened without change of diagnosis.
His forehead is very red and bumpy, he says that it has a burning sensation and feels uncomfortable. Also, the skin changes rapidly in terms of texture and pimples, for days it can be fine and then suddenly worsen. He notices that when he changes the room where he sleeps his symptoms can improve or get worse.
His arms have red spots that are itchy and he scratches them every day.
Is this maybe scabies on hands and rosacea on the face or something else some allergy?
Case Detail
30 yo man comes in for a non-healing wound. 8 months prior he dropped a heavy piece of metal at work and it struck his right shin. He was wearing pants but the skin was nonetheless torn open. He works in a grain mill and said it is common to have dust get under his clothes.
The initial wound was approximately 2 x 0.5 cm (black arrow), but it never healed properly. Several months later he noticed insidious onset of itchy erythematous papules. They seem to be clustered for the most part on his right shin, although he has several papules on his left leg/thigh as well. The spot near his ankle was especially bothersome (as you will see by the excoriations).
Additionally, he did have a rash on his glans which was quite itchy as well. He was adamant that the penile rash developed along with the leg rash and denied risk factors for STI.
I did an x-ray to rule out an underlying bony injury or radiodense foreign body. They looked perfectly normal (not pictured).
I am honestly quite stumped by this one. I was thinking of perhaps a foreign body reaction, an insidious bacterial infection, or perhaps a fungal infection. I referred the patient to dermatology for further evaluation, but was hoping to plumb the collective wisdom of Figure1. Any ideas from the experts?
