A 16-year-old previously healthy, morbidly obese male patient presented in my office with a 2-day history of mild sore throat and one day history of skin rash on his lower legs which started that morning and during the day expanded to involve upper legs, abdomen and lower arms (no image available) which is "burning" in quality. He has no other symptoms. His PHx and SEHx are unremarkable.

On physical exam I noticed this purpuric rash which in his lower legs has necrotic appearance with central spots of pustules. On other areas is only purpuric. He didn't have mucosal lesions. The rest of the exam was unremarkable.

I gave a provisional diagnosis of necrotic vasculitis (leukocytoclastic type) and sent the patient to the dermatologist. I was also thinking about Henoch-Schonlein purpura.

What is your opinion?

1.5/52 history of a developing rash in an 87yo Female she was picked up at home by us and taken to local ED. on legs and hands majority however has started to appear on rest of body. Rash itself is purpuric in areas, hot to touch, causing all over body pain. Rash is blanching in areas. And tender when palpated. Patient has NKDA and no other allergies that she was aware of. Patient has no Pyrexia and no other observations have been present. Patient has cellulitis in her left leg which was treated with fluclox for 7/7 3/52 before. Does anyone have any ideas to diagnose.


60 year old patient, female. Eruption appeared two days after assuming allopurinol. Rash is very itchy. They definitely look like hives but have a more purpuric look to them...could it be because the patient is under warfarin? Other hypothesis, urticaria vasculitis?


37yo Asian male C/O skin rash with swelling at Rt lower leg after went camping over the weekend 3 days ago. Reports some itching w/o pain. Denies fever/chills, night sweat, N/V, dyspnea/SOB. Denies bite. He took some cephalexin which helps for swelling. PMH/SH- unremarkable. Meds- none. Exam is positive for rash at Rt lower leg (nonblanced w/ pressure. Afebrile with normal VS.


A 48-year-old male patient with a history of stable psychosis (under medication) came to my office because of a 2-day history of stingy and little tender rash on his left sole and slight swelling of his left lower leg. He didn't have any other symptoms. On exam, I noticed this pustular rash on the left sole, as one open blister on 2nd finger and one tense painful close with thick roof on 3rd finger. His lower left leg has slight swelling and erythema, but it was not painful. The rest of the exam was unremarkable. His PHx was positive for 2 episodes of erysipelas on the left leg, so he thought that this was the 3rd attack. His therapy: clozapine, fluphenazine, diazepam. I ordered lab analyses which showed normal CBC and CRP of 8.9 (ref. <3.0). I discarded erysipelas/cellulitis as a diagnosis and referred the patient to a dermatologist. What is your opinion on this case? What could this be?
