35 yr old male slipped while walking the dog.
Injured his ankle and came to the ED. Teaching points:
- pain at lateral malleolus, unable to bear weight more than 4 steps (Ottawa Ankle rules).
- AP view shows shortened fibula (Dime Sign), mortise view shows ankle incongruity (Shenton's line) and increase medial gutter (>4mm).
Similar Cases
59yom "twisted" ankle then largely unable to bear weight.
Haematoma evident below lateral malleolus but not unstable. ?? Avulsion # @ talar insertion of talofibular ligament? Edges not typically as sharp as expected but it does correlate clinically. Other views appear entirely normal (?). View Case

C/o Left Ankle Mild Pain around Peroneal tendon (occasional) and Right Ankle Mild Pain at Ankle-Foot joint.

AP (image 1) and lateral (image 2) radiographs of the ankle show multiple injuries.
The ankle mortise is disrupted with medial displacement of the tibia and widening of the space between the tibia and fibula. In addition, there is an oblique fracture of the distal fibular. The proximal fibular is also displaced medially. On the lateral view, the tibia and fibula are displaced anteriorly and there is a fracture (arrow) of the posterior surface of the tibia. This injury pattern represents a trimalleolar fracture. View Case

Patient in late teens presenting after collapse.
- No chest pain, no previous cardiac history, no history of sudden death in family, are there any features in this EKG that would warrant further work-up or is this just a pediatric EKG? View Case
