c/o SOB that started 1 night prior. Pt denies URI symptoms but does admit to hearing some wheezing. Pt is able to speak in full sentences to the author. Pt admits SOB is at rest. She has a PMHx of HTN/HLD/PAD/PAF/II block/PP.

Upon ROS, pt ADMITS Wheezing, R ear pain, and R LE Edema. Pt denies author symptoms of no chest pain, chest tightness, any history of prior "blood clot" in leg or lungs, and no recent illness requiring prolonged bedrest.

She sleeps on two pillows just to make her feel comfortable and denies having to increase them to help with breathing, no claudication, abd distention, no frothy sputum. Pt denies hx of asthma, COPD, but endorses use of an inhaler in the past to treat her bronchitis but states this was a long time ago.

Pt is following cardio and EP; however, she is overdue for an appointment.
