9 yr old girl presented with cough for 5 days and SOB for 2 days. associated with pleuritic chest pain, diarrhea and vomiting. no edema no hemoptysis no night sweats no weight loss.

examination: generally she looks ill, tachypnic and uses accessory muscles.

Vs: pulse:140 Rr;40 Temp;35.6. oxy;85%.

resp exam: decreased breath sound on the right side and dullness to percussion.

HEENT: raised JVP

cardiac: normal

CBC and CXR was done. CBC shows high WBC and high Granulocyte but normal lymphocytes. HB was 10.8 MCV 79.

what is your differential diagnosis and management??
