Syncope
Mid 50's M called for dizziness with syncope x1. Onset was sudden about 15 minutes prior to calling. No recent nausea, vomiting, diarrhea, bleeding, or illness. Hadn't taken night time BP meds. He didn't know the reason for thinner use.
Complaints: dizziness, syncope, chest pain, slight shortness of breath
Hx: Back pain, sciatica, HTN (meds included HCTZ, Metformin, apixaban, and a few others I don't recall).
Vx: GCS 15, HR 130, SpO2 93%, BP unobtainable (auto/manual/palp) and only once registered 87/34, EtCO2 18, RR 30, Temp LO, BGL 205.

So the PT went into cardiac arrest on the CT table. CT showed a saddle PE.

63 year old female A&Ox4 with a C/C of syncopal episode while sitting. +weakness, -dizziness, -chest pain, -SOB. +nausea/vomiting after second witnessed syncopal episode. Hx of bilateral #Mastectomy due to breast #Cancer, no cardiac hx and no cardiac meds. Initial BP 100/60 sitting.
64YOM called EMS due to fall. Pt found #Glasgow-Coma-Scale 15/15, dizzy lying on floor unable to self mobilise. Pt tachy with HR- 130, RR 40. BP 78/43. On questioning pt is Hep C+, has had 4/7 heamoemisis (black blood as pointed out in 2/3 photo) with 1X episode of frank blood today, causing dizziness + fall. Pt managed as #Hypovolemia, cannulated, IV fluids(approx 350ml) until dizziness dispersed. With legs elevated + fluids BP 92/55 HR-100 RR-26. Transported to ED, flounder with #Hemoglobin of 6.8g/100ml, treated with blood transfusion in resus.
